This is perfect for our church. But if we don't write the date on it right away - we lose track of WHEN the visitor was at the church. That's the only thing this is missing to make it an excellent product, asking the right info even the email address of the person. Jut add a reminder for the person to put the date and it's perfect!!! Thanks so much.

I'm glad that I found this site. Very professional looking and the price is right. My pastor was very pleased with everything including the wording. I wouldn't change a thing.
